It really depends on the establishment for me. If it's a locally owned place or a small franchise, I would likely give the change back. However if it's some big name place and/or the employees are rude, then it's their loss.
I've really only had this happen once that I can remember, and it was at Best Buy. The chick at the register was so busy yapping to one of her friends in line behind me that she forgot to ring up about $30 worth of stuff. I noticed, but I kept my mouth shut and told her to have a nice day as i walked out.
