Do suit pants usually come pleated? Can they be flat front? If pleated, can the pleat be taken out?

They can be either. It depends on the look you are going for. I think most men look better in flat front pants. I wouldn't take the pleats out because the front would be baggy. Just buy flat front if that's your preference.

My gf is adamant I try and get them tailored to be flat front (if that is even possible). I don't mind either way and it's less hassle for me to leave them with the pleat.

I have both, but most of mine are flat. More than likely your suits are gonna have to be tailored anyway, but I'm not sure you can eliminate a pleat.
