This is more than likely your situation. The DR. is not in your network, therefore they send YOU the check and you're responsible for paying it.

not sure, but if you dont sign paperwork at the dr office assigning benefits to the dr, the ins co sends the money to you, idk

Sometimes rates change even if it's a little bit and your insurance co. will adjust the premium. If you are on the positive side, you get the reimbursement. When you call in for changes to your policy, that's also when you may get a check, or a bill. Regardless either way, you should get an endorsement statement from them explaining the change.
